Evolve
New patients for physical therapy and
rehabilitation services are typically scheduled
within 24 hours. Most insurance plans require
some type of pre-authorization and we prefer
to verify your benefits for physical therapy prior
to your first visit.

A prescription for physical therapy and
rehabilitation services is required from a
physician, physician's assistant, nurse
practitioner, dentist podiatrist or chiropractor.
If you have seen your physician or other
health care professional, we will be happy to
contact their office and obtain a copy of your
physical therapy referral for you.

appointment or have any questions, you may
email or call the appropriate person from the

Due to our work schedules, we check our
voice mails and email in the morning,
mid-day and at the end of the day. Please
allow sufficient time for us to respond by
